Vendor Information & Application
At the Eastham Farmers Market, we are committed to supporting local agriculture and food producers. Vendors may sell agricultural, horticultural, or food items that they have personally grown, produced, or processed. Below please find details on permits and vendor approval, please contact the Market Manager or Health Department for questions.
Prior to applying, please read the Rules and Regulations, and Board of Health Requirements for the permit type you are applying for.

- The Vendor Fee is $25.00 per market
- Vendor Space for 10 weeks is $225.00 per 10X10 space or $112.50 for 5 weeks per 10x10 space

Vendor Applications Types and Requirements
Farmers & Growers Limited
- Whole, uncut fresh fruits and vegetables
- Unprocessed honey, or raw honey as defined by the National Honey Board
- Pure maple products
- Farm fresh eggs (stored and maintained at 40°F or less)
- Cut Herbs
- Potted Herbs
- Cut Flowers
Farmers' Market application only.
Inspections to be conducted by Health Department staff, inspections may or may not be scheduled with vendor.
Farmer & Grower Expanded
- Shellfish
- Fin Fish
- Live Lobster
- Meat
- Poultry
- Dairy
Shellfish or Finfish
- Copy of valid current retail seafood permit and/or retail seafood truck permit issued by the Department of Marine Fisheries (DMF)
- Written Standard Operations Procedure detailing how the product will held under required time and temperature controls
- Hazard Analysis Critical Control Points (HACCP) plan which details food safety precautions
- Shellfish: Harvest area with GPO coordinates
- Shellfish tags must be retained by vendor for all product for sale at the Farmer's Market, and be available on site for inspection
- Copy of current vehicle registration, if seafood retail truck
- Pre-operational inspection by the Eastham Health Department
Live Lobster
- Copy of valid current retail lobster permit and/or retail truck lobster permit issued by the Department of Marine Fisheries (DMF)
- Written Standard Operations Procedure detailing how the product will held at required time and temperature controls
- Hazard Analysis Critical Control Points (HACCP) plan which details food safety precautions
- Copy of current vehicle registration, if lobster retail truck
- Pre-operational inspection by the Eastham Health Department
Meat or Poultry
- Meat: Must be slaughtered in a federally inspected facility. Copy of license issued by the Food Protection Program required.
- Poultry: Must be processed in a USDA facility or state only licensed facility. Copy of facility license required.
- Product to be held frozen. Only frozen product will be permitted to be sold.
- Product must be properly labeled.
- Written Standard Operations Procedure detailing how the product will held at required time and temperature controls
Milk or Milk Products
- Sale of raw/unprocessed milk or milk products for human consumption is prohibited
- Cheese must be processed in a licensed manufacturing facility. Copy of facility license required
- Product must be properly labeled
Inspections to be conducted by Health Department staff, inspections may or may not be scheduled with vendor.
Health Department may impose additional requirements and may prohibit the sale or preparation of some or all Potentially Hazardous Foods (PHF)
Processed & Prepared Foods
Processed/Prepared Foods or Prepared On-Site Vendors
- Breads & Baked Goods
- Cheese
- Jam & Jellies
- Ciders
- Juice
- Maple Syrup
- Granola
- Other Prepared item
- Copy of current year Food Establishment Permit issued by Health Department in municipality that the base of operations is located
- Copy of current Servsafe Certificate
- Copy of current Allergy Awareness Certificate
- Copy of Menu (if applicable)
- List of Prepackaged food items for sale (if applicable), required to be labeled with the following:
- Name of product
- Name and address of the manufacturer
- List of ingredients, including potential allergens
- Notice if item is produced in a facility that handles potential allergens
- Net weight
- Expiration date/sell by date</ul>
- Sketch of floor plan of vendor site, sketch to include the following:
- Location of all food prepared tables/areas
- Location of refrigerators/coolers
- Sampling display areas (if applicable)
- Single serve storage areas
- Location of generators or electrical hook up (if applicable.

Mobile Food Unit
- Mobile Food Unit/ Food Truck
- Copy of Base of Operations Food Establishment Permit issued by Health Department in municipality that the base of operations is located
- Copy of Menu
- List of Prepackaged food items for sale (if applicable), required to be labeled with the following:
- Name of product
- Name and address of the manufacturer
- List of ingredients, including potential allergens
- Notice if item is produced in a facility that handles potential allergens
- Net weight
- Expiration date/sell by date
- Copy of current Servsafe Certificate
- Copy of Allergy Awareness Certificate
- Copy of current vehicle registration
- Floor plan of mobile unit
- Location of all food prepared tables/areas
- Location of refrigerators
- Sampling display areas (if applicable)
- Storage
- Location of propane tank
- Location of electrical components
- Location of fresh and waste water storage tanks
- Confirmation of current Mobile Unit inspection by the Cape & Islands Fire Prevention Association
